While caring for patients during these first two weeks of clinical, it is extremely important to provide safe and effective care. As healthcare workers our goal is to establish a trusting relationship and set goals for patients to improve their health and advance their lifestyle choices. To start the day, my preceptor and I look over the schedule and discuss common diseases that she sees daily. Typically, in the primary care office hypertension, diabetes, hyperlipidemia, and anxiety are the most common. Occasionally patients will come in to be cleared for a procedure and require certain blood work, ECG, and physical prior to the surgical date. The population consist of patients in early adolescences to geriatrics.

Professional behavior in the practicum setting is extremely important. Patients seek healthcare providers to establish care, but also feel safe and trusted with their health, wellbeing, and concerns. Dressing appropriately, introducing yourself, and listening to the patient with effective communication verbal and or nonverbal can establish a trusting relationship. Thus far for clinical, patients have allowed me to perform physical exams and contribute to their care with my preceptor. Afriyie (2020) states, ?effective communication has been linked to improved quality of care, patient satisfaction and adherence to care, leading to positive health outcomes? (Afriyie, 2020, pg. 438). A patient came into the office to establish with a new provider to answer her questions and concerns. The patient had a strong family history of thyroid cancer and with her recent blood work, was told everything was normal from her previous physician. However, her thyroid labs were abnormal, and the patient was extremely anxious and tearful requesting assistance with her care. My preceptor and I listened to her and addressed her concerns. We sat and listened to her by providing verbal and nonverbal communication skills. The patient thanked us multiple times for allowing her to voice her concerns and stated, ?thank you both for really listening to me and calming my nerves?. By easing her anxiety and providing effective communication skills, she gained trust with us. I am excited to see the patient as she returns in a month to see how they are feeling with medication and diet changes.

Many patients are geriatric or on disability that come into the office with Medicare/Medicaid as his or her primary insurance. Some medications are only covered if other trials have been tried and failed for full coverage. Thankfully, the office has samples and rebate cards that can be given to the patient to help decrease the cost. Having access to available resources to assist with accurate diagnosis and treatment plans can help improve the patients care. In the computer system epic, there are many tools that can provide the patient with education with their diagnosis. We were able to print information on diabetes and how to properly provide finger sticks. Frost (2020) states, ?shifting care to primary and community care rather than specialist care settings potentially provides more holistic care closer to home that is also able to draw on multiple professional inputs? (Frost et al., 2020, pg. 2). My preceptor can see when the patient last saw his or her podiatrist, ophthalmologist, endocrinologist, and so forth all within the system ?care everywhere?. By seeing other specialist visit notes, this allows her to continue providing effective care towards the patient and see what the provider has recommended the patient. She currently reviews medications with the patient in hopes to prevent polypharmacy.

Collaborating with the preceptor and multidisciplinary healthcare team to provide and promote the best health outcomes is the goal for the patient but also healthcare providers. ?Care planning assists professionals to communicate information about the patient?s care to others, to facilitate continuity of care. Communication may be predominantly verbal, but it will also always involve documentation in a variety of formats, including computer based, handwritten or preprinted care plans? (Ajibade, 2020, pg. 1169). In the office, a behavior specialist works side by side with the providers and can assist with patient visits, whether face to face, telemedicine, or phone calls. Many patients who use the multidisciplinary team member can discuss certain topics such as pain management, mental health concerns, or request refills for controlled substance medications. As mentioned earlier, in the computer system, care everywhere, allows healthcare workers to view documents with one another and communicate if necessary.

In conclusion, to provide the best care it is done in a safe and effective manner. Communication is important between the patient, healthcare providers, and multidisciplinary team members. As healthcare providers, we seek to improve our patient?s health and lifestyle changes. By including the patient in their plan of care and setting realistic goals, we hope to see improvement and achievement. Providing plans and communicating effectively can improve patient satisfaction and trust.
